Third Party Service Billing
Dear Parents: To accommodate Third Party billing (billing to an organization or a person who is providing lessons that are part of your child's educational plan), Anchor has developed a form and a procedure to help in the process. This process must be followed to obtain funding from the student grant for the lessons.1. Contact your teacher as soon as you have a need for third party billing.
2. Your teacher will authorize the lessons and send you a signed form (like the one at this link below) usually by email or fax. She will also fill in the proper PLO codes that are required at the bottom of the form.
3. You can then fill in the rest of the form and submit it to the third party who in turn sends it to Anchor as a bill for the service. Even if the third party has their own bill to send this form must be sent to the office along with that bill or you can send it in separately by fax or scanning and emailing.
4. If the third party requires immediate payment the form will direct them to call Robin at the Anchor office who can accomodate the request (usually by providing a credit card number).
