Third Party Service Provider Invoice - PLO numbers

On page I-19 in the Anchor Academy 2007-2008 handbook, you will find the Third Party Service Provider Invoice. This invoice can also be accessed through Anchor Academy's webpage under the link to "Forms".

Use this invoice when you are using part of your child's funding to pay for lessons in sports or fine arts. The service provider needs to fax this form to Anchor Academy in order to receive payment. At the bottom of the invoice is a space that asks for the PLO#s that the service provided meets. Below are the PLO #s that you can use to fill in this space.

PE - Sports lessons including swimming, team sports, skating, skiing, karate, etc.: Kindergarten and Grade 1 - PLO#s - 1, 2, 6, 8, 9, 10 Grade 2 and Grade 3 - PLO#s - 1, 2, 6, 7, 8, 9, 10

Music - Music lessons including piano, guitar, violin, voice, etc.: Kindergarten and Grade 1 - PLO#s - 1, 2, 3, 4, 5, 6, 7, 8, 9 Grade 2 and Grade 3 - PLO#s - 2, 3, 4, 5, 6, 7, 8

Dance: Kindergarten and Grade 1 - PLO#s - 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11 Grade 2 and Grade 3 - PLO#s - 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11
